Portal: FAQ/Troubleshooting

If you wish to create a patient portal account please contact our office to receive your activation code or ask about the patient portal at your next appointment.

If you already have an account and are having trouble logging in please see if any of the following troubleshooting suggestions help:

  • If you forgot your login name, click “Forgot login name?” under “Sign in” and you will receive a portal reset letter in the mail.
  • If you have an account and know your username, please enter your username and click on “Forgot password?” under “Sign in” and fill in the email associated with your account as well as your login name. You will be emailed further instructions. 
  • If you are having difficulty with either of these steps, once clicking the “Forgot login name?” or “Forgot password?” there are video tutorials at the top of these pages.  Please click on “View a video tutorial" for step by step instructions

If you are still having difficulty accessing your patient portal account and have tried the above, please contact our office at 716.650.3000 and press option 0 to speak with a medical secretary.
